The public enquiry form collects only the details needed to respond to non-urgent appointment enquiries, referral questions and clinic administration. This may include your name, contact details, enquiry type, preferred contact method and a brief message.

Please do not include DOB, NHI, uploaded documents or detailed clinical history in the public form. The clinic will advise how any further information should be provided.

Information submitted through the form is used by the OrthoDocs clinic team to review and respond to your enquiry. Email and web forms may not be suitable for sensitive clinical information.

Do not use the public form for urgent clinical concerns. For medical emergencies, call 111. For new or worsening symptoms requiring urgent assessment, please contact your GP, an urgent care provider, or Whanganui Hospital Emergency Department.
